FD's mission is "to ensure that children and families have the fundamental assets they need to thrive and live independently."Job SummaryThe Front Desk Associate performs a variety of tasks, as s/he is the first contact by the patient and families with the clinic. Essential duties include providing excellent patient services via phone and in person; referring callers to the appropriate parties; making appointments for patients according to established protocols. The position is considered a crucial link between the patient and the care delivered by clinical staff. The incumbent must work collaboratively with all Clinical staff in support of direct patient services, exhibiting flexibility and a "can do" attitude. Patient services are the key priority in this position requiring the Receptionist to serve as a point of contact with other internal and external departments, all with the goal of fostering an environment which promotes patient comfort and trust. The position must exemplify the core values and mission of the organization, always exercising utmost discretion, diplomacy and tact in patient/staff interactions. Major Responsibilities/Activities

  • Facilitate the exchange of health information and documentation of care received by the child in the community, including attending any community-based care.
  • Coordinate Medicaid services that the child receives in the community and transitioning the child to community services as part of discharge planning. This includes maintaining and tracking medical referrals for specialty care and procedures outside of Forestdale's Clinic; tracking and ensuring follow up by patient/family.
  • Collect reports from external medical providers, facilities, and previous placements.
  • Liaise with outside providers to ensure change of status for patients is communicated (e.g. trial discharge, final discharge, etc.)
  • Coordinate care with school-based health providers, when applicable.
    • Create and manage health records in the electronic health system, and other agency data clearinghouses, maintaining all documentation and data entry standards to ensure regulatory and best practice compliance.
    • Schedule appointments and maintaining calendars/appointments for psychiatrist, clinic providers and dentist
    • Maintain and track mental, dental and medical referrals and follow up
    • Coordinate and resolve medication inquiries from pharmacy and patients
    • File and fax documents
    • Answer calls and deliver messages / calls to appropriate persons
    • Make reminder calls for clinic appointments
    • Order and maintain office supply inventory
      • Other duties, as assigned Minimum Requirements
        • Associate / Bachelor's degree
        • Minimum of three years of work experience in the field of mental health or child welfare.
        • Strong organizational, verbal and written communication skills
        • Proficient in Microsoft office
        • Ability to multitask
        • Medical Assistant or LPN certification preferred, but not a must
        • Spanish-speaking preferredBenefits
