Company DescriptionThe purpose of Assertive Community Treatment (ACT) is to deliver comprehensive and effective services to individuals who are diagnosed with severe mental illness (SMI) and whose needs have not been well met by more traditional service delivery approaches. ACT is an evidence-based practice that provides an integrated set of other evidence-based treatment, rehabilitation, case management, and support services delivered by a mobile, multi-disciplinary mental health treatment team to support recipient recovery through a highly individualized approach that provides recipients with the tools to obtain and maintain housing, employment, relationships and relief from symptoms and medication side effects. The ACT Team Leader is responsible for the administrative, clinical, and fiscal operations of the ACT team.Our TeamWe recognize that each CASES staff member is critical. We are a team strengthened by our diversity of backgrounds, perspectives, and talents and united by a common purpose.We are determined to continuously enhance our knowledge and skills, evaluate our results, and learn from our successes and failures.Our Impact On The CommunityWe believe public safety is best protected through alternatives to incarceration that combine accountability and support with empowerment and opportunity.We are committed-through continuous innovation and the strategic disruption of ineffective responses to crime, addiction, and mental illness-to creating solutions for some of the city's most difficult public safety and public health challenges.Reports to: Senior Director, Mobile Behavioral Health ServicesLocation: Central HarlemJob Description* Operate program with high fidelity to the NYS OMH ACT regulations * Ensure all clinical documentation is completed in a thorough and timely manner* Remain in compliance with licensing and practice standards set forth by OMH and DOHMH* Provide clinical oversight to the team, including person-centered treatment planning, evidence-based practices, use of service dollars, high risk clinical rounds, and incident management.* Achieve expected outcomes (e.g., reduced use of hospital services) for ACT clients through medication, health, substance use, employment, family, wellness, and peer supports* Provide training, clinical supervision, evaluation, and professional development opportunities to all staff* Maintain fiscal viability of the program through Medicaid billing & budgeting of program fundsQualifications* Active New York State license: LCSW, LMSW or LMHC* At least 2 years of experience working with individuals with serious mental illness, with expert-level knowledge of relevant evidence-based practices* At least 2 years of supervisory experience, preferably overseeing a multidisciplinary team* Knowledge of the ACT model, the Footholds AWARDS electronic health record, and proficiency in Spanish are preferredAdditional InformationSalary: Competitive salary, fulltime 35-hour week with excellent benefits.Only applicants selected for interviews will be contacted.CASES is proud to be an Equal Opportunity Employer.
