The Medical Outcomes Manager, Strategic Partnerships engages Healthfirst sponsors, providers, community and public health organizations to address gaps in clinical performance and care outcomes. Working closely and in alignment with Delivery System Engagement, Population Health, and Clinical Quality, this role emphasizes improving practice level performance to address gaps in efficiency, effectiveness, and the patient experience.Interacts with hospitals, providers, and other stakeholders through structured face to face and "drill down" meetings, collaboratives and workshops to improve practice and population level performance against Healthfirst utilization, quality and patient experience standardsPresents actionable aggregate and practice level data as a foundation for offering technical support recommending process improvement, monitoring progressImplements and facilitates initiatives and workshops to promote practice performance improvementOffers regional and targeted provider symposia in collaboration with DSE and Quality to promote implementation of evidence based and evidence informed processes and clinical pathwaysWorks under light supervision, maneuvers multiple tasks independently with a fast-paced proactive vs. reactive approach to changing priorities while meeting departmental objectives.Documents Clinical Partnership and Healthfirst databases and systems per department standards and organization protocolsManages relationships and effectively communicates to internal and external partners regarding objectives and targetsSubject Matter Expert (SME) for all relevant Partnership performance reports (including but not limited to utilization, risk score, access to care, patient experience, and as relevant, HQIP, HEDIS and STARS) that are disseminated to external partnersCollaborates with key departments on initiatives championing performance improvementAnalyzes data to identify data-driven opportunities for improvement and determine root causes in areas that need improvement at the individual site, practice or network level to identify trends, issues, and opportunities for performance improvementPrepares memoranda and presentations that succinctly and accurately summarize complex information, details and dataAdditional duties as assignedMinimum Qualifications:Ability to travel around the NYC, Long Island, Orange, Rockland, Sullivan and Westchester counties/regionBachelor's degreeIntermediate MS Word, Excel, PowerPoint, SharePoint, Version One or Project Planning softwareExcellent oral and written skillsGood presentation, relationship management, and analytic skillsPreferred Qualifications:Master's DegreeBackground in health care or managed careStrong time and project management, critical and creative thinking, communication, writing, and problem-solving skillsUnderstanding of healthcare data elements and metricsPlease note: Since we care so greatly about our employees' and members' wellbeing, Healthfirst is moving to an environment where our employees are fully vaccinated against COVID-19.
