Job Description:

Intro/Program Description: Goddard Riverside's five-story residence on West 140th Street provides affordable, permanent supportive housing for 48 previously homeless adults with mental health diagnoses. The building consists of fully furnished studio apartments, offices for the social service and building staff as well as ample common space, including a Recreation Room, kitchen, library and backyard. On-site services include case management, entitlement assistance, medical and psychiatric care, substance abuse counseling, work readiness training, other employment related groups, and connection with ACCESS-VR. 24 hour/7 day per week front desk security, and group activities.
Purpose of Position: To deliver effective case management services using a strengths-based, client-centered approach to formerly homeless adults diagnosed with a severe persistent mental illness in order for tenants to successfully maintain permanent housing and optimal well-being.
Schedule: Monday through Friday - 9:00am-5:00pm
Goddard Riverside and Isaacs Center follow the CDC and NYS recommendations to prevent the spread of COVID-19, and are now requiring all new hires to be vaccinated against COVID-19 unless they have a qualified exemption.
1. Client Service

  • Provide case management to a group of assigned clients; identify short and long-term treatment goals in collaboration with client and help clients achieve them, including linkages to community programs such as medical, psychiatric and drug treatment services.
  • Maintain all necessary statistics and required documentation of services included assessments, psychosocial, service plans and progress notes
  • Track and maintain all data outcome measure on tenant's physical, mental and behavioral health with a result orientated framework
  • Assist clients in receiving and maintaining entitlements
  • Assist clients in understanding and complying with building rules and government subsidies to maintain residency
  • Accompany clients on visits to doctor's entitlement offices or other community services, in person and Virtually
  • Serve as liaison between other service providers and community resources
  • Support tenants during emergency crisis events and contact first responders as necessary
  • Encourage participation among resistant clients; conduct meetings to introduce and encourage program participation
    2. Participate as a West 140th Team Member
    • Participate in weekly staff meetings, case conferences, training programs and supervision meetings.
    • Conduct apartment meetings
    • Other duties as required in accordance with agency polices and funding source requirements, including incident reporting, data collection, and audit preparation
      Qualifications/Educational Requirements
      • Bachelor's Degree required
      • Prior experience with homelessness preferred
      • Experience working with individuals diagnosed with severe and persistent mental illness, substance use and/or co-occurring disorders preferred
      • Prior case management experience preferred
      • Bilingual preferred (English and Spanish)
        Skills, Knowledge and Abilities
        • Understanding of housing first model
        • Understanding of severe and persistent mental illness
        • Understanding of harm reduction and trauma informed care
        • Strong interpersonal skills
        • Strong written and verbal skills
        • Team player
        • Organized with good time management
          Computer Skills Needed: To perform this job successfully, an individual should be:
          • Proficient in Microsoft Office
          • Able to use or learn computer databases such as AWARDS and Paycom
            Physical Requirements
            • Some walking and travel to escort clients to various appointments throughout the city and occasional visits to hospital emergency rooms.
            • Visits to tenants' onsite apartments is required
              Work Environment
              • Mostly indoor office setting. Regular visits to client apartments are also required.IND123
