    About the Role:
    • The Head of Care Management Operations is part of Cityblock's Care Services Operations team and reports directly to the Chief Operating Officer. This role is responsible for:
      • Builds a Care Management Center of Excellence that maintains expert knowledge of current industry standards, improvement activities, and strong care management execution skills
      • Works with both internal and external stakeholders to promote an understanding of care management operations and objectives within the company and to prioritize departmental projects according to Cityblock goals
      • Collaborates with other leaders in developing, monitoring, and evaluating Care Management improvement action plants, year-round internal monitoring, and processes management
      • Monitors and reports care management measures by contract, centers of Medicare and Medicaid (CMS), and accrediting requirements
      • Ensure the execution of care management model of care related activities to achieve operational efficiency, clinical effectiveness, and overall positive outcomes
      • Must develop strong working relationships with local market leadership to ensure delivery of all program services are in accordance to the model design
      • Participate and provide feedback to the strategy of the overall program design and be responsible for execution of changes in the model, workflow, metrics in assigned markets
      • Conduct data analysis, request data when needed, to optimize performance based on data results
      • Assist in the design, participate and maintain knowledge of all care management and systems utilized to deliver all program based on market level and front line team experiences
      • Launch new market entry specifically for the care management programs. This requires the hiring of a local care management team along with training and education with the local leadership, providers, care team members on all related programs
        Requirements for the Role:
        • 5-10 years in Care Management for Medicare and Medicaid, and other government program operations
        • 5+ years Case/Care Management experience
        • 5+ years of previous management/supervisor level experience
        • Experience with a start-up environment, design and/or care management programs redesign and execution
        • Demonstrate execution and management of complex care management programs with successful track records and results
        • Strong process improvement skills and operational mind-set
        • Requires BA/BS in clinical or health care field (i.e. nursing, health sciences, public health)
        • Previous experience in working with NCQA, HEDIS, Care Management Operations
