POSITION SUMMARYThe Treatment Apartment Program (TAP) is a Medicaid-billed program serving individuals living with severe and persistent mental illness ("members"). This program serves our members and works to assist individuals in reaching their independent living goals. The role of the Case Manager works with our members to assess their individual goals and devises a person-centered plan that reflects their independent living objectives. Our commitment is to support members in maintaining in safe, stable, and affordable housing.


  • Support a case load of 8- 10 individuals living with severe and persistent mental illness (SMI)
  • Provide one-on-one support with individuals living in TAP, including weekly in person face-to-face meetings
  • Build strong and genuine relationships with individuals in living in TAP
  • Conduct weekly residential visits to our scattered site apartments located in Brooklyn, Bronx, or Manhattan
  • Ensure the apartment passes necessary health and safety checks for maintenance, cleanliness and perform monthly fire drills
  • Ensure that all TAP members have paid their monthly housing fees
  • Devise Individualized Service Plans and Service Plan Reviews regularly, according to duration of services
  • Provide de-escalation and problem-solving strategies for members in crisis, as needed and in accordance with the OMH standard and the FH social practice model
  • Partner on a weekly or monthly basis with social practitioners, care managers, Home and Community Based Service (HCBS) supports, physicians and therapists
  • Develop strong relationships with landlord/building management companies to advocate for members to maintain safe and working housing facilities
  • Facilitate discharge and move - out operations and procedures including transition planning and coordinating the physical move
  • Perform other related duties as assigned by supervisor, such as rotating involvement in our Housing Resource CenterAdministrative Duties
  • Maintain and update extensive files according to state New York State Office of Mental Health (OMH) regulations, policies, and procedures
  • Develop working relationships with landlord/building management agencies
  • Maintain compliance with FH, city, and state (OMH) guidelines
  • Correspond and collaborate regularly with various benefit organizations such as Social Security and Medicaid, hospitals, non-profit providers, community care
  • Maintain accurate and organized files to adhere to strict deadline requirements as deemed by New York State Office of Mental Health (OMH) on a weekly, monthly, quarterly, and yearly basis
  • Document all services provided by writing progress notes, Service Plans and Service Plan Reviews regularly in AWARDS
  • Develop and implement individual discharge/transition planning
  • Collaborate with TAP Manager regularly on program and professional developmentREQUIRED KNOWLEDGE, SKILLS, AND ABILITIES
  • Excellent verbal and written communication skills, including ability to effectively communicate within the Agency and with external providers such as Physicians, Social Security, Medicaid, landlords and other building management personnel
  • Expertise in navigating the New York City Transit System
  • Excellent computer and software proficiency (MS Office - Word, Excel, and Outlook)
  • Experience with AWARDS database preferred, but not required
  • Must be able to work under pressure, in a crisis, meet documentation deadlines, while maintaining a positive attitude and providing support in a respectful empathic manner
  • Ability to work independently, multitask and to conduct assignments to completion within parameters of instructions given, prescribed routines, and standard accepted practicesREQUIRED EDUCATION, CERTIFICATION, AND CREDENTIALS
  • Bachelor's degree required
  • 1-3+ years of experience working with individuals with mental health conditions, substance use disorders or who experience homelessness
  • Positive, energetic, dynamic, and engaging personality
  • Driver's license preferred, not required PHYSICAL REQUIREMENTS
