Work with the largest care delivery organization in the world and start doing your life's best work.(sm)The Value-Based Program Coordinator is a key member of the Population Health Market Operations team. Critical to the success of this work is the 1) accurate and timely documentation of patient level risk factors, including any existing chronic conditions, inside payer based platforms and 2) identification of services that have been delivered to close outstanding gaps in care on an as needed basis. We are seeking a candidate with the ability to learn how to use these tools by incorporating relevant information from Electronic Medical Record (EMR) systems. Candidates must demonstrate effective time management skills to ensure overall department goals are met. The person in this role is expected to work collaboratively with oher Coordinators on the team and reports to the Value-Based Program Supervisor. Primary Responsibilities:

  • Complete, review and submit risk score capture forms for traditional Medicare, Medicare Advantage and certain Commercial value based programs on corresponding payer platforms
  • Download and transmit to payers relevant supplemental materials from various EMR systems to close outstanding gaps in care for certain clinical measures
  • Participate in annual MIPS Quality reporting
  • Participate in annual PCMH reporting
  • Collaborate with team on adhoc data/record collection throughout the year
  • Collaborate with Market Operations outreach team to engage value-based patients with no record of prior annual wellness visits

    • High School Diploma or equivalency
    • 1+ years in medical office or similar environment
    • Proficient with Microsoft Office applications including Outlook, Word and Excel

      • Bachelor's degree
      • Medical/clinical background
      • Knowledge of electronic medical record systems, Epic software
      • Knowledge of coding abstraction
      • Solid written and verbal communication skills
      • Ability to work independently and to troubleshoot and resolve issues in an expedient manner
