Family Health Centers at NYU Langone, a designated Level 3
Medical Home, is the largest federally qualified health center

opportunity to join our team as a Coordinator of Therapy Services.
In this role, the successful candidate develops and implements
support and education related to cardiopulmonary rehabilitation.
Will perform examination, assessment, care planning, and treatment
related to cardiopulmonary rehabilitation. Delegates and monitors
responsibilities assigned to support staff and students.
Participates in performance improvement activities and in all
mandatory staff education programs. Functions as a clinical liaison
between in-patient medical service and out-patient pulmonary
programs to identify appropriate rehabilitation candidates.
Position is critical to identify post-COVID and long-hauler COVID
recovered patients.Job Responsibilities:
- Responsible for maintaining daily census of pulmonary
patients.
- Responsible for screening all referrals for pulmonary
rehabilitation needs.
- Responsible for tracking all outpatient pulmonary
rehabilitation patients on master census.
- Responsible for compliance with CMS regulations.
- Responsible for tracking various departmental data
metrics.
- Provides clinical oversight and support for pulmonary
rehabilitation staff and interns.Assists with orientation and
development of new pulmonary rehabilitation staff and interns.
- Participates in interdepartmental performance and quality
improvement activities.
- Functions as a Liaison between in-patient medical service and
out-patient pulmonary programs to identify appropriate
rehabilitation candidates.
- Monitors and develops new quality improvement projects for
pulmonary rehabilitation.
- Provides appropriate educational instruction to patients,
patient caregivers and other clinical staff.Additional Position
Specific Responsibilities:Other: 1. Good computer skills.2.
Excellent oral and written communication skills.3. Knowledge of and
experience with Managed Care and health insurance procedures and
systems4. Knowledge of local, state and federal health care
regulatory, licensing and accreditation procedures and
requirements.5. Language skills, in addition to English,
preferred.6. Ability to track and trend data related to
cardiopulmonary rehabilitation via Excel or other formatsMinimum
Qualifications:To qualify you must have a Education: Graduate of a
professional Masters/Doctorate degree program in Physical Therapy.
License, Certification, Registration: New York State License or
eligible in Physical Therapy CPR certification AACVPR certification
preferred. Required Licenses: -Heart Saver/First Aid/CPR/AED
Certificate, Physical Therapist License-NYSPreferred
Qualifications:Work Experience: Experience as a Physical Therapist
in a hospital or health care setting is required. Three to five
years experience with progressive responsibility within hospital
setting.Qualified candidates must be able to effectively
