Job Description:
Overview
Responsible for providing comprehensive primary care, coordination
of 24 hour care, medical coverage, and directly provides services
to ArchCare Senior Life participants. Demonstrates the knowledge
and skills necessary to assess, plan care for and provide service
to frail elderly participants according to assigned
responsibilities and Arch Care Senior Life standards. As a
physician member of the Arch Care Senior Life, Inc. team, the
Primary Care Physician must meet all federal and state licensing
laws, certification and regulations as necessary.
Responsibilities
Essential Functions--- Assumes responsibility for the medical care
of all participants assigned to the ArchCare Senior Life
interdisciplinary team.--- Provides face-to-face comprehensive
History and Physical assessments on new referrals. Prepares
face-to-face Quarterly or as needed History and Physical
assessments on established participants--- Provides evaluation and
treatment of participants during acute illness--- Determines the
need and facilitates referrals to other medical specialists when
deemed necessary--- Discusses care guidelines with participants.---
Judicious utilization of specialist consultations--- Rotating night
and weekend call with other ArchCare Senior Life primary care
providers.--- Participates in the development and revision of the
participant's plan of care as a member of the interdisciplinary
team. Integrates the primary care treatment plan into the overall
plan of care developed by the interdisciplinary team. Interacts
with other team members to meet emergent and acute needs of
participants. Participates in discharge planning.--- Participates
in quality management program activities including peer review.---
Documents changes in the participant's condition and details care
provided by completing all required PACE documentation and ensuring
compliance to Agency standards and policies including required
hospital and nursing home documentation and medical oversight---
Collaborates with nurse, nurse practitioner and other physicians to
ensure comprehensive and continuous care.--- Provides acute care
inpatient consultations (e.g.: as medical consultant to ArchCare
Senior Life participants on non-medical services.--- Maintains
clinical privileges at contract facilities and completes all
requirements for ongoing credentialing and Continuing Medical
Education at contract facilities and with ArchCare Senior Life---
Assists in monitoring the medical care of enrolled members who are
hospitalized in out-of-network and out-of-service area facilities,
and assists in their transfer to in-network facilities when
medically appropriate.--- Provides on weekend, holiday and on-call
coverage periodically as per the medical coverage schedule,
including hospital admissions, clinic hours at the center, and
nursing home, emergency room or home visits to avoid unnecessary
hospitalization.--- Keeps abreast of federal outcomes-based quality
improvement activities and geriatric standards of care. Keeps up to
date on issues pertinent to chronically ill populations and end of
life issues, pain management standards and other issues applicable
to the clinical population being served.--- Knows and adheres to
the philosophy and goals of ArchCare Senior Life--- Maintains a
quiet and safe environment for participants, visitors and staff.---
Keeps confidentiality of participant records, reports and
discussions--- Participates in formulation and maintenance of
ArchCare Senior Life policies and procedures.--- Participates in
ArchCare Senior Life Committees as requested by the Medical
Director and Executive Director.--- Advises the Medical Director in
ways and means to establish better accountability of ArchCare
Senior Life services to clients and referral sources.--- Attends
and participates in scheduled staff meetings and participant care
meetings as requested.--- Maintains flexibility in schedule and
responds to unexpected emergencies and changes in workload in order
to fulfill responsibilities--- Utilizes supplies and equipment
economically.--- Informs the Medical Director of "unusual
incidents".--- Identifies service delivery problems and uses good
judgment in their solution.--- Is professional in appearance and
manner in the clinical area; recognizes own limits and seeks help
and guidance from the Medical Director as appropriate; responds in
a positive manner to constructive criticism; serves as a role model
for students and staff members--- Optionally holds membership in
professional organizations--- Maintains applicable licensure and
certification and pursues professional growth through continuing
education process.--- Performs other related duties as required
Qualifications
Education Requirements--- MD degree, completion of residency in
Internal Medicine or Family Medicine.
Experience Requirements--- Minimum one (1) year post graduate
experience in Internal Medicine or Family Medicine.
Experience Desired--- One year of experience with a frail geriatric
population in a home care, long-term care, PACE and/or MLTC
setting.
License Requirements--- MD, DEA, licensed to practice in the state
of New York. Board certified in Internal Medicine or Family
Practice.
License Desired--- Certificate of added qualifications in
geriatrics and/or experience in the care of chronically ill and
frail seniors or interested to pursue as part of professional
planning
