Apply now!DUTIES:Obtains evaluates and utilizes diet history information for planning the nutritional care of dialysis patients.Records appropriate information in the medical record including evaluation of nutritional status, reaction to and comprehension of diet instructions and notes on the nutritional progress of the patient.Reviews patient's medical chart checking lab values for trends and correlations.Calculates and document nutritional intakes and dietary patterns.Participates in health team rounds assisting the physician in establishing the proper diet order according to the patient's specific problems and requirements.Acts as a consultant and resource person to physicians, nurses and other members of the health care team in the area of renal diets as well as in all areas of normal and therapeutic nutrition.Converses with patients to ascertain any specific problems that need attention as it relates to diet.Meets with dialysis patients to ascertain and ensure proper dietary requirements/restrictions are being met.Counsels the patient and his/her family on the rationale of the diet and/or meal plan that has been developed.Conducts in-service education for other members of the Renal Dialysis Health Care Team.Complies or develops education material and uses them as aids in nutritional education.Participates in continuing education opportunities in order to update knowledge and keep abreast of changes in diet therapy in ESRD and other related fields.Interprets, evaluates and utilizes current research related to nutritional care.May perform other duties as deemed necessary by the Nurse Manager.REQUIREMENTS:Be a registered dietitian with the Commission on Dietetic Registration.Have a minimum of 1-year professional work experience in clinical nutrition as a registered dietitian.Rate: $70,000 - $85,000LOCATIONS:Brooklyn, NY170 Buffalo Ave51 Georgia Ave144 Walworth St.DIAL23ABOUT US:With five centers, located throughout the Tri-State Area, Centers Health Dialysis' facilities have been providing quality care for over 13 years to our dialysis community.
