Job Description:
You'll be providing services in the New York City boroughs to provide excellent care for people experiencing homelessness as part of the Street Health Outreach and Wellness project (SHOW) along with Advanced Practice Practitioners and certified health providers. This means you will support the wellness and recovery goals of adult patients facing chronic and complex conditions (e.g., co-morbid medical and mental health diagnoses, limited personal resources, chronic medical conditions) in the field. Targeted Case Management services are tailored around a patient's life goals and designed to provide high quality care.
- - Our LMSWs engage in assessments and provide treatment for patients facing substantial barriers to accessing the traditional healthcare system.
- - Each day, you will meet your team at a designated location and be in contact with other clinical team members, and patients' other medical providers to coordinate optimal care and resources for the patient consistent with state regulations and company standards and policy.
- - This is a roving job, where you'll be on your feet for at least 6 hours a day.
- - You'll be supported along the way by your team, with medical direction, and by the Clinical Practice Group which provides our NP's and PA's clinical and non-clinical administrative support. -
- - SHOW is a new model of mobile units from NYC Health + Hospitals aimed at providing COVID-19 tests and vaccinations, wound care, basic material necessities, and harm reduction services to New Yorkers who are unsheltered.

Responsiblities:

The goal of the social worker is to help patients regain optimum health or improve functional capability given the patient's setting.
Engage in assessments, create a treatment plan including referrals.
Each day, you'll maintain patient medical records and medical documentation consistent with state regulations RCN standards and policy through our HealthPoint system.
Participate in continuing education as required by the state where you reside.
Other related duties as deemed necessary.

QUALIFICATIONS

New York State Licensed Master Social Worker
Knowledge of community resources and counseling / social work practices with high-risk populations

PREFERRED QUALIFICATIONS

Experience working with patients who are homeless or in emergency department environments
