Job Description:

Cardiology opening in New Hyde Park, New York. A long-term care facility on Long Island is actively seeking a compassionate and patient-oriented Nurse Practitioner / Physician Assistant (NP / PA). In this role, the Nurse Practitioner / Physician Assistant (NP / PA) will be responsible for the delivery of compassionate and quality medical care of assigned residents in coordination with other members of the healthcare team. Responsibilities The Nurse Practitioner / Physician Assistant (NP / PA) will:

• Examine residents and deliver quality care in varied settings to include subacute, long term, rehab and outpatient care
• Order diagnostic workups and tests and timely completion of medical records and discharge summaries in a timely manner
• Assist in resident/patient/family education and re-examine, re-evaluate and modify care plans as necessary, including maintaining proper documentation in resident charts
• Prescribe medication as necessary
• Review all diagnostic testing results and make recommendations to findings
• Ensure Encounter forms are completed accurately and in a timely manner
• Educate and supervise rotating medical and physician assistant students
• Participate in on call monthly/weekend medical coverage Qualifications
  • Active New York State NP, ANP, GNP, or PA license
  • 2+ years of experience in a sub-acute or long-term care facility administering medical care to residents
  • Graduate of an accredited Physician Assistant or Nurse Practitioner program
  • Proficient in SigmaCare or another EMR (Electronic Medical Record) software
