Job Description:
This facility will be Manhattan's first Transition to Home Unit
(THU) which is a new 25-bed inpatient unit. Referrals will come
directly from selected New York City Health and Hospital CPEPs. The
unit will care for individuals experiencing homelessness and
provide intensive, recovery-oriented programming focused on
developing skills for successful community living. Comprehensive
discharge planning services will ensure housing and wrap-around
services are in place at the time of discharge. The mission of the
THU program will be to restore dignity and mental wellness to the
New Yorkers whom it serves by offering ethical care and
individualized supportive services in a recovery-oriented
environment.
