Position Summary
The Clinical Coordinator (RN) will support patient care and
education in an outpatient clinical setting in various
hematology/oncology clinical specialties.
Responsibilities
* Establish and maintain a therapeutic relationship with patients
and their families and work as a liaison between the physician and
the patient/family for home and hospice care.
* Understand and integrate cultural diversity into practice.
* Develop and implement patient education materials as it relates
to diagnosis and treatment of Cancer.
* Perform comprehensive patient assessment; offer patient and
family education and symptom management related to disease and its
treatment.
* Act as part of a multidisciplinary team to provide continuity of
care throughout various stages of treatment and follow-up.
* Provide compassionate, evidence-based nursing care to oncology
patients and families living with cancer in a healing and
innovative environment.
* Communicate with NP and physician regarding patients in clinical
practice and attend multidisciplinary rounds weekly.
* Communicate and collaborate to integrate evidence based practice
and research findings into care of the patients.
* Collaborate with Infusion Center nurses and staff to establish
nursing plan of care and to select the best interventions to
achieve desired patient outcomes.
* Answer patients' call and triage them based on symptoms and
severity.
* Perform additional related duties as needed or directed.
Minimum Qualifications
* Bachelor's degree or equivalent in education and experience, plus
two years of related experience
* Current New York State RN license
* BLS Certification
Other Requirements
* Participation in medical surveillance program due to contact with
patients and/or human research subjects and potential bloodborne
pathogen exposure
* Successful completion of applicable compliance and systems
training requirements
