Please find the job details:Title: Care Manager EDLoation: New York, NY 10004Shift: 9-5 Hybrid, 40 hrs a weekContract: 13+ weeks with possible extension*Required: *Bachelors Degree required; BSN preferred.Two (2) to three (3) years clinical experience in certified home health agency (CHHA), Lombardi program and/or MLTC or MAP program*Skills:** *Five years minimum of clinical experience as a Registered Nurse.** *Leadership experience preferred. Experience with Care Coordination, Utilization review and discharge planning Computer literacy required.**Education:*1. RN Licensed as a Registered Nurse in the State of New York required.2. Baccalaureate degree required.3.
