**Wellness Nurse Job Description**

Provide high-quality care to residents, ensuring their overall well-being and promoting a safe and healthy environment.

Job Responsibilities

• 
  
• Identify ongoing needs and services of residents, promoting the highest quality resident services.
  
• Ensure proper follow-through and documentation for residents with changes in healthcare services and status.
  
• Assist in conducting resident evaluations and gathering data at move-in, 30 days post-move-in, and semi-annually.
  
• Conduct monthly wellness visits for all residents.
  
• Monitor and follow up with residents who have experienced falls, accidents, or incidents, documenting and reporting as required.
  
• Document all pertinent information in the resident wellness file.
  
• Maintain communication with residents' families and responsible parties to ensure medical needs are met.
  
• Contact residents' attending physicians when necessary and/or upon family request.
  
• Ensure weights and vital signs are obtained monthly for each resident.
  
• Assist in maintaining wellness files according to Sunrise policies and federal and state regulations.
  
• Maintain medical supplies and emergency kits for the community.
  
• Provide clinical support and assistance to community team members as needed.
  
• Understand and follow infection control practices.
  
• Partner with the Resident Care Director and community team to promote infection control standards.
  
• Regularly monitor each resident's medication and treatment profile to ensure accurate administration and documentation.
  
  
  Qualifications
  
  • 
    
  • Graduate of an approved college or school of nursing.
    
  • Current state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN).
    
  • Minimum of one year experience in home health, assisted living, hospital, or long-term care environment.
    
  • Demonstrate knowledge of nursing practices, techniques, and methods applied to health and wellness resident care.
    
  • Demonstrate knowledge of good assessment skills.
    
  • Ability to handle multiple priorities.
    
  • Possess written and verbal skills for effective communication and the ability to teach and guide team members.
    
  • Competent in organizational, time management skills.
    
  • Demonstrate good judgment, problem-solving, and decision-making skills.
    
  • Demonstrate proficiency in computer skills, Microsoft Office, and Sunrise applications.
