This position would be responsible for providing skilled nursing care to homecare patients in Newark, Livingston, Millburn and Short Hills as needed.   • Supports organization's mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
  • Assesses and evaluates client for physical therapy needs and develops and implements appropriate plan of action in collaboration with the physician
  • Documents all changes in client's condition and updates care plans and outcomes accordingly
  • Identifies counseling needs and plans appropriate teaching strategies
  • Monitors quality and appropriateness of services provided
  • Adheres to agency infection control and safety policies
  • Adheres to the organization's policy in regards to absenteeism and appearance

  • Physical Therapy Licensure in NJ required
  • 2-3 years of prior clinical experience and community based experience preferred
  • Strong computer skills
  • Strong communication and interpersonal skills
