Choose from 3 clinics in and around Queens, Brooklyn & the Bronx.OVERVIEWValue based care for seniors in a neighborhood clinic setting where classes are offered, transportation provided if necessary, meaningful doctor relationships & the welcoming atmosphere feels like home.Will see patients in 100% outpatient clinic with onsite labs. Care providers team at each location is 3-5 (mix of NPs and Primary Care Physicians)ROLESchedule: Monday-Friday 8am-5pm'Avg. 12-14 patients a day' 40 minutes with each new patient' Patient's care team includes You, PA or NP, Scribe, MA, Social Worker'Monday thru Friday- No nights, No weekends!Community room patient activities: Exercise, events, yoga, dance, spa daysCustom EMR softwarePACKAGEBase Salary range $220-$250k dependent on experienceIncentive bonus on top of base- determined by quality metricsPartner track6 weeks total holiday, PTO & CME$5000 Continuing Medical Education stipendHealth, Vision, Dental, and Life Insurance401K with 4% company match, vested immediatelyMedical Malpractice Insurance
