Job Description:

emotional support to patients recovering from illness, injury or
surgery. With a comprehensive range of services supported by an

surroundings of their home. - Manages the delivery of high-quality
home care services to patients consistent with agency standards,
policies, and state and federal regulations by providing leadership
and guidance to nursing staff - Ensures adequate staffing and
appropriate patient assignments - Maintains the integration of
interdisciplinary services - Facilitates the achievement of agency
goals related to clinical outcomes, patient experience of care,
fiscal responsibility, and staff retention - Graduate from an
accredited School of Nursing; BSN preferred. - Current NYS RN
license and registration required. - Minimum of 3 years’ experience
in a Certified Home Health Agency; supervisory experience
preferred. - OASIS E; clinical outcome measures. - Federal
Conditions of Participation and NYS DOH regulations related to Home
Health Care. - STAR ratings
