Job Description Intake RN – Managed Long Term Care (MLTC) -
$20,000 sign-on bonus ($5,000 payable every six months, less taxes)
- Immediate Full Time Opportunity Location: Bronx, NY Monday-Friday
(No Weekends) Salary Range: $95K- $105K As an MLTC-Intake RN, you
will assess potential new members for eligibility and explain the
program to them and their families. You will play a crucial role in
ensuring a seamless transition to managed long-term care. Key
Responsibilities: Conduct outreach activities within the community
to assess the eligibility and safety of potential members. Will be
doing 2 visits a day in the Bronx only. They will be either in the
home or telehealth. Collaborate with the Intake/Outreach team and
clinical team to ensure appropriate services are provided to new
members. Communicate effectively with potential members, families,
referral sources, and physicians. Qualifications: Current New York
State Registered Nurse License. Associate’s Degree in Nursing
Required. Bachelor of Science in Nursing (BSN) Preferred. One (1)
year of experience in an acute care setting. MANAGED LONG TERM CARE
EXPIERENCE IS A MUST (MLTC) Must have UAS-NY experience. Home care
and management experience preferred. Proficient in MS Office.
Strong oral, written, and interpersonal communication skills.
