Job Description UAS Nurse – PACE Program /nLocation: Bronx, NY
/nSalary: $100,000–$120,000 (DOE) /nSchedule: Full-Time | Day Shift
/nOverview /nA large, well-established Medicaid/Medicare PACE
program with multiple locations throughout New York City is seeking
an experienced UAS Nurse to join its growing clinical team. This
organization is dedicated to helping seniors receive nursing
home–level care while continuing to live safely and independently
in their homes and communities. /nThis role is based in the Bronx

package. /nKey Responsibilities/n /n/t - Conduct pre-enrollment UAS
assessments for prospective participants using strong clinical
judgment and nursing expertise /n/t - Determine clinical
eligibility and assess whether candidates meet criteria for PACE
services /n/t - Perform comprehensive clinical, functional, and
environmental assessments across service areas /n/t - Develop
initial and prospective plans of care based on assessment findings
/n/t - Complete all required documentation, pre-screening forms,
and assessment paperwork accurately and timely /n/t - Conduct
semi-annual reassessments as required /n/t - Make plan-to-plan
calls when indicated /n/t - Evaluate appropriateness of therapy
services for new participant referrals /n/t - Participate in
interdisciplinary and cross-functional meetings /n/t - Maintain
flexibility in scheduling and respond to changes in workload or
urgent needs /n/t - Travel throughout assigned service areas to
complete assessments /n /nQualifications/n/n /n/t - Active New York
State Registered Nurse (RN) license /n/t - Bachelor’s Degree in
Nursing (BSN) required /n/t - Prior experience conducting UAS
assessments strongly preferred /n/t - Strong assessment,
documentation, and care coordination skills /n/t - Ability to work
independently while collaborating with interdisciplinary teams /n
