Why This Role Stands Out - Monday–Friday, 9:00 AM – 5:00 PM - 35-hour workweek – strong work-life balance - Mix of site-based and in-home care - Collaborative interdisciplinary team (IDT) environment - Serve a frail and elderly population with comprehensive support What You’ll Do - Evaluate participants and develop individualized occupational therapy treatment plans - Provide hands-on therapy in clinic settings and participant homes - Collaborate closely with providers, IDT members, and specialists - Document evaluations, progress notes, discharge summaries, and care plans in the EMR - Recommend adaptive equipment and support safe discharges - Participate in care plan meetings and performance improvement initiatives - Provide clinical guidance to COTAs and support OT students as needed - Maintain compliance with all clinical, safety, and regulatory standards What We’re Looking For Required Qualifications - Bachelor’s degree from an accredited Occupational Therapy program - Active New York State Occupational Therapist license - Ability to travel locally within Brooklyn to participant homes and sites - Strong communication, documentation, and clinical judgment skills Preferred Experience - At least 1 year of OT experience, ideally with geriatric or frail populations - Familiarity with community-based care or PACE-style programs - Experience using EMRs (Care Compass a plus) - Bilingual skills are a plus, depending on participant population Physical & Professional Readiness - Ability to lift up to 50 lbs and assist participants safely - Comfortable with standing, walking, bending, and in-home visits - Up-to-date immunizations and medical clearance required - Commitment to ethical practice and ongoing professional development Location Brooklyn, NY Services provided at program sites and participant homes within the borough.
