Job Description Job Description Overview of the Registered
Dietitian RD Position: The Registered Dietitian (RD) plays a vital
role in evaluating and tracking the nutritional health of residents
at our facility. This position involves creating personalized care
plans in partnership with medical professionals, ensuring adherence
to dietary standards, and providing education to both residents and
staff. Collaboration with interdisciplinary teams, including
speech-language pathologists, is essential to enhance the overall
health and well-being of residents in our skilled nursing and
rehabilitation environment. Our organization is seeking a
Registered Dietitian (RD) in Brooklyn, NY. RESPONSIBILITIES:
Evaluate and monitor the nutritional status of residents, offering
guidance to medical personnel and formulating appropriate care
plans. Assess the unique nutritional needs of residents,
particularly concerning nutrition support, skin integrity, and
significant weight fluctuations. Oversee meal services for
residents to ensure adherence to dietary modifications. Provide
education to residents, families, and staff regarding nutrition and
dietary adjustments. Regularly review Dietary Care plans for
effectiveness to maintain high standards of care. Participate in
weekly meetings with the interdisciplinary team to assess and
modify residents’ care plans as needed. Collaborate closely with
Speech-Language Pathologists to ensure residents with dysphagia
receive suitable mechanically altered diets. Examine all menu
modifications to ensure compliance with facility policies and
State/Federal regulations. Ensure adherence to regulations and
policies regarding weight monitoring within the facility. Engage in
daily communication with the interdisciplinary team to deliver
quality care to residents. QUALIFICATIONS: Must hold a Registered
Dietitian credential. Strong interpersonal and leadership
abilities. Familiarity with relevant state and federal regulations.
Proficient in computer skills, including EMR software and menu
