Job Description:

Brooklyn’s residents. The role of the Medical Social Worker is to
work with the interdisciplinary team to ensure a safe discharge of
all adult patients. Duties include but are not limited to: ·
Identifies patients with barriers to a safe discharge plan. ·
Completes psychosocial assessments on all patients. · Documents all
clinical assessments, notes, and referrals in Epic. · Completes
screens and coordinates all new skilled nursing facility and rehab
facility referrals. · Assesses all cases involving child
abuse/neglect, domestic violence, elderly abuse, and sexual
assault. · Educates patient/family about post-acute care options
and the role of choice. · Educates patient/family about managed
care and payor issues This is a full-time day shift position
9:00am-5:00pm Monday-Friday including every 4th weekend, both
Saturday and Sunday, located at NewYork-Presbyterian Brooklyn
Methodist Hospital. Preferred Criteria - At least one year of
experience in an acute care setting - Discharge planning experience
- Experience with Epic and Allscripts Requirements: - Master’s
Degree in Social Work (MSW) - New York State License as a Licensed
Masters Social Worker (LMSW). - Communication and negotiation
skills - Ability to work independently and collaboratively with the
interdisciplinary team. - Work efficiently and effectively under
pressure. - Work comfortably within the limitations of the medical
