Registered Nurse (RN) | Home Health Location: Brooklyn, NY Agency: Cynet Health Pay: $2,350 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 260 Weeks Start Date: 1/22/2026 About the Position Job Title: Registered Nurse Profession: RN Specialty: Home Health Duration: 260 Days Shift: 5x8 Days Hours per Shift: 8 Experience: Minimum of 2 years in a home health setting License: Current RN Licensure in Client Certifications: BLS required before the start date Must-Have: - Two (2) years of experience in a home health setting within the last fifteen (15) years. - Recent experience conducting comprehensive assessments of patients. Description: We have an urgent need for a dedicated Registered Nurse in the home health specialty. The ideal candidate will possess a current RN licensure in Client. This position requires a minimum of two years of experience in a home health setting within the last fifteen years. Candidates must have recent experience conducting comprehensive assessments of patients. The role involves evaluating health status, strengths, care needs, and preferences to guide the development of individualized long-term care service plans. You will review consumer medical documentation and health referral forms as relevant to each case. Data from evaluations must be entered into an electronic format and transmitted as required. Documentation of concerns or conflicting information that arises during evaluations is essential. Continuity of care is emphasized to reduce fragmentation and gaps in treatment plans. You will discuss health care options and necessary supports with consumers. This position includes initiating communication with consumers for enrollment into a managed care health plan. Enhancing communication and collaborative relationships with interdisciplinary care team members is crucial for care coordination. The RN will collect quality review data and necessary documentation for outcome measurements and record case notes. Identifying health promotion and illness prevention opportunities is a key responsibility. A comprehensive working knowledge of community resources and payor requirements is required. The nurse must meet all performance standards established for this position. Additional duties may be assigned by management as needed. All candidates must be prepared to conduct in-field assessments.
