Registered Nurse (RN) | Case Manager Location: Brooklyn, NY Agency: Cynet Health Pay: $3,035 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 9 Weeks Start Date: 3/9/2026 About the Position TravelNurseSource is working with Cynet Health to find a qualified Case Manager RN in Brooklyn, New York, 11212! Job Title: RN Case Manager Profession: Nursing Specialty: Case Management Duration: 13 weeks Shift: Days Hours per Shift: 8 hours (5x8) Experience: Minimum three (3) years in direct patient care required. License: Current RN License required. Certifications: Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) required. Must-Have: - Bachelor’s Degree in Nursing (BSN) required. Description: Coordinates and ensures the delivery of care. Collects utilization management and quality assessment data. Regularly interacts with physicians to clarify plans of care. Reviews patients for high-risk criteria for discharge planning needs. Monitors patients and determines needs for a timely discharge. Confers with third party payers within 24 hours past notification. Conducts initial case review no later than 72 hours after admission. Screens patients to determine if they meet high-risk criteria for discharge planning needs. Develops discharge plans for uncomplicated cases. Makes referrals to Social Workers for assessment and discharge planning as indicated. Collaborates with other members of the healthcare team to facilitate the progress of care. Ensures that a plan of care is developed expeditiously by the attending physician. Ensures that a plan of care is time-oriented and has clearly defined objectives. Negotiates, if needed, with physicians and other healthcare practitioners to develop a more effective care plan. Monitors duplication of tests, delays in diagnostic, treatment, and/or scheduling. Ensures documentation of patient progress toward identified clinical outcomes by healthcare practitioners. Conducts concurrent reviews. Participates in the collection of data related to performance improvement. Reviews cases for continued stay appropriateness. Collaborates with physicians regarding issues. If unsuccessful, refers to a physician advisor. Accurately documents review findings and provides clinical information to third party payers. Performs retro reviews as requested by third party payers. Maintains current knowledge of developments in the field. Reviews professional literature to seek out and recommend improvements. Maintains competency by participating in in-service and continuing educational programs. Maintains membership in professional organizations, and attends lectures, and reads journals and articles. Develops and maintains a high level of patient and staff satisfaction. Displays courtesy, tact, and diplomacy when dealing with staff, colleagues, superiors, patients, visitors, and vendors. Offers assistance to those mentioned above in attaining their goals or completing their tasks whenever possible. If unable, obtains appropriate assistance. Performs other related duties assigned by manager or supervisor. In an emergency, performs other duties necessary for the welfare of patients or the efficient operations of the institution.
