Job Description Our client is seeking a Care Manager, with experience providing professional intervention, and critical services to clients. Care Manager duties consist of assessment, crisis, intervention and financial assistance. Responsibilities: Conduct comprehensive assessments to identify the social, emotional, and financial needs of clients and their families. Advocate for clients by connecting them with appropriate community resources and support services. Perform regular home and field visits with clients, adhering to contract requirements and working across multiple assigned sites. Maintain accurate and up-to-date client records in both physical files and electronic databases. Provide ongoing case management services, including eligibility assessments and service coordination. Develop, implement, and monitor individualized service plans to deliver a full range of social service interventions. Collaborate with team members and partner agencies to ensure clients receive timely and effective assistance. Qualifications: Bachelor’s degree in Human Services, Sociology, Psychology, or a related field (or equivalent experience). Minimum of one year of experience in a social services or case management setting. Strong organizational, communication, and computer skills. Must be able to conduct home and field visits and travel throughout the five boroughs. Full-time schedule: Monday – Friday.
