seeking a compassionate Geriatric Nurse Practitioner to deliver
high-quality, patient-centered primary care to residents in
assisted living communities throughout New York , using a hybrid
model that combines on-site care and telehealth . ? Why You’ll Love

care. Excellent Pay Flexibility $135,000 – $160,000 per year (based
on experience & schedule) W2 and 1099 options available Travel

comprehensive health assessments and physical exams Conduct
medication reviews and manage treatment plans Create personalized
care plans with an interdisciplinary team Manage chronic and acute
conditions with skill and compassion Order and interpret labs and
diagnostic tests Prescribe medications within NP scope of practice
Educate residents and families on health, wellness, and
independence Maintain accurate documentation in eClinicalWorks
Participate in quality initiatives that genuinely improve care What
You’ll Bring ? Master’s degree from an accredited Nurse
Practitioner program ? Active New York State NP license and DEA
registration ? Eligible for Medicare/Medicaid billing ? 3 years of
clinical experience ? Geriatric or long-term care experience
strongly preferred ? A compassionate bedside manner, a big heart
