Job Description Soloh Partners is seeking a local contract nurse RN Home Health for a local contract nursing job in New York, New York. Job Description & Requirements - Specialty: Home Health - Discipline: RN - Start Date: 02/09/2026 - Duration: 13 weeks - 40 hours per week - Shift: 8 hours - Employment Type: Local Contract Description: Job Responsibilities: Duties may include but are not limited to: -Assessing, planning, implementing, and evaluating patient care plans in consultation with healthcare professionals. -Evaluating medical information, as well as providing direct and individualized bedside nursing care to pre-and post-surgery patients. -Preparing, administering and recording prescribed medications. -Changing dressings, inserting catheters, and starting IVs. -Adjusting specialized medical equipment as needed, as well as interpreting and recording measurements. -Monitoring patients' vitals and reporting adverse reactions to medications in a timely manner. -Collaborating on discharge planning with the healthcare team. -Answering questions and educating patients and their families on surgical procedures and post-surgery care. -Maintaining and updating patient files, medical charts, and health records. -Adhering to best nursing practices, standards, and protocols. . Skills: Required Skills & Experience: -Acute Facilities: Minimum two (2) years Med/Surg experience in a large acute care hospital setting. The most recent experience should be within the past year. -Post Acute Facilities: Minimum two (2) years Med/Surg experience in either Post Acute Facility or acute care hospital setting. The most recent experience should be within the past year. -Physical assessment. -Medication administration. -Wound care & Infection control -IV therapy. -Post-operative care. -Patient education. -Telemetry monitoring. Preferred Skills & Experience: -EPIC EMR experience. Required Skills & Experience: -Two (2) years of previous acute care experience in MedSurg, Maternal and Child Care, Critical Care, Pediatrics. -Current Adult Care home health experience.
