Option for 3 days per week Friday to Sunday What You Will Be Doing Conduct home visits and manage all aspects of patient care in compliance with regulatory standards Complete required documentation, including 485 forms, interim orders, and medication profile updates Communicate with primary care providers and coordinate care with other disciplines Review and complete case communication notes and follow-up items in HCHB Participate in case conferences with supervisors and interdisciplinary teams Maintain accurate and timely records to ensure quality and compliance Skills You Need Active Registered Nurse (RN) License in New York Strong clinical assessment and documentation skills Ability to work independently and manage multiple cases Excellent communication and interpersonal skills Proficiency with electronic health record systems (HCHB preferred) Interested in hearing more?
