children, youth, families, seniors and homeless prevention.
BronxWorks is seeking a Case Manager to join a diverse and
enthusiastic team at Cooper Gardens, a multi-building supportive
housing program located in the Bronx, NY (walking distance from
Arthur Avenue.) Cooper Gardens provides services and permanent
housing to 95 formerly homeless households, consisting of both
single individuals and families with children. Work schedule is
Mon-Fri from 9am-5pm and staff are required to work one Saturday
per month from 8am-4pm. RESPONSIBILITIES: Maintaining a caseload of
12-18 households (individuals and families) with chronic
homelessness history Monitor and document client progress
towardsserviceplan goals Conduct regular case conferences with
clients and service providers as needed Outreach clients, and
conduct monthly home visits,and field work,with clients when needed
Assistwith managing and directing all walk-in clients Maintain
familiarity with agency and city resources available to clients
Document progress notes for all encounters and services provided to
clients Participate in regular meetings, trainings and weekly
supervision with supervisor and other support staff Completing
ongoingServicePlans, Apartment Inspections,and Assessments for
caseload Coordinate and connect clients to variety of medical,
mental health and behavioral health physicians and services
Assessing ongoing eligibility for various entitlements and
resources for caseload Meeting monthly reporting requirements
includingtimelydocumentation and engagement with full caseload
Assistingclients with Representative Payee services including
budgeting and financial education Participate intheMedication
Monitoring Training(MMT)Program, become certified, and provide
these services to clients on-siteon a daily,weeklyor as needed
basis Become SOAR-Certifiedor complete any similar training to
complete SSI/SSDI applicationsfor caseloadin a timely manner.
Support programbyassistingwith client events, as needed Work one
Saturday shift,from 8am-4pm,eachmonth Performadditionalduties as
assigned QUALIFICATIONS: Proficiencyin modern business
communications including personal computers, electronic mail,
voicemail, facsimile and copier equipment Bachelor’s degree in
social work or another related field of study One year of case
management or relevant work experience preferred Engagement and
detailed oriented skills are essential Proficiencyin Microsoft
Office suite and other standard business technology isrequired
Strong oral and written communication, timemanagementand
organizational skills are necessary PHYSICAL REQUIREMENTS Ability
to use a computer for prolonged periods. Ability to occasionally
lift and/or move up to 10 pounds. Ability to stand, walk, or sit
forlong periodsof time. Ability to bend and retrieve objects and/or
documents. Ability to travelinthe boroughs of New York City and its
adjacent counties via public transportation. Ability totravel fora
minimum of 1 mile within a reasonable amount of time and in all
types of weather, including inclement weather, ifrequiredto conduct
