field-based position will cover the following locations: Manhattan,
Bronx, Brooklyn, Queens, and Long Island. MJHS is a large
not-for-profit health system in the Greater New York area. Our

and palliative care programs offer a significant difference when
dealing with a life-limiting condition. We offer a broad range of
services in the community or facility-based to help patients and
their families ease the burden of managing a life-limiting illness
or end of life care. The Pediatric NP Provider Hospice/Palliative
Care has two primary responsibilities. - He/she will provide
NP-appropriate service to patients enrolled in hospice and may also
assist in staffing pediatric palliative care consultation services
that function as a program of MJHS Hospice and Palliative Care -
He/she will work in the community environment, and may, on
occasion, provide consultation in the long-term care environment or
hospital. - In all settings, he/she will provide in-person visits
or telehealth visits, as appropriate. - Providing comfort care
consistent with best practices, including history-taking,
appropriate examination, formulation and care planning, treatment
recommendation or implementation, communication, and documentation
- In the role as hospice NP, he/she will coordinate with the team
physician and manager and will be responsible for face-to-face
evaluations for hospice eligibility, and the assessment and
management of hospice patients in need of exigent medical care -
He/she will also contribute to the training of the hospice team,
providing support for the interdisciplinary team, as needed -
He/she will also help build a teaching curriculum on pediatric
aspects of hospice care that will be of use to our hospice workers
who are engaged in pediatric care through cross coverage for when
young adolescents are incorporated into adult hospice teams - In
providing both direct care and support to other clinicians, the NP
will use an evidence-based approach to management and to the use of
materials and policies/procedures - Other activities related to
quality improvement projects may also be required, as appropriate •
Master’s degree in nursing and graduate of an approved Nurse
Practitioner Program required. • Minimum of 1-year clinical
experience as a NP provider preferred. • Prior experience working
as a member of a specialist-level palliative care team or working
in a hospice environment preferred. • Prior pediatric nursing or
pediatric NP experience strongly preferred. • NYS NP License
(Certified Pediatric, Family, or Palliative Care Nurse
Practitioner) • CPR-BLS • NYS Driver's license and car • Hospice &
Palliative Care certification strongly encouraged by year 2 of
hire. • Microsoft Word and Excel • Ability to work flexible hours
to meet the needs of the position • Ability to coordinate the
resources and personnel involved in a comprehensive
interdisciplinary team environment. • Ability to travel across the
agency’s geographic territory to provide patient care as needed. •
Possess teaching, communication, and listening skills. • Ability to
work non-judgmentally with patient / family members of any culture,
religion, socio-economic background, or lifestyle. • A high degree
of accountability • Excellent communication, listening, and
organizational skills • Ability to work within an integrated health
care team • Ability to demonstrate effective critical thinking
