Job Description:
Monarch Rehab, a leader in personalized rehabilitation services,
is looking for a Licensed Practical Nurse (LPN) to join our dynamic
team. This individual will play a crucial role in the delivery of
our physical, occupational, and speech therapy services, tailoring
care to each patient’s specific needs. Your contributions will
directly enhance patient outcomes and improve quality of life
across our community. Administer medications and treatments as
prescribed by the healthcare provider. Monitor, record, and report
symptoms or changes in patients' conditions. Maintain accurate
health care records and reports. Provide compassionate care that
aligns with patients’ rehabilitation goals. Assist with patient's
daily living activities and physical therapy exercises. Participate
in the assessment, implementation, and evaluation of individual
patient care plans. Collaborate with multidisciplinary team members
to facilitate integrated and comprehensive care. Exceptional
patient care and assessment skills. Strong communication and
interpersonal skills. Ability to administer medications and perform
treatment protocols. Proficient in maintaining accurate patient
records. Capable of handling emotional and physical stress related
to patient care. Medical Insurance Dental Insurance Vision
Insurance Current LPN licensure in the state of practice. CPR/BLS
certification is required.
