Job Description The Quality Management Outreach Specialist / Medicare Sales Representative supports health plan quality improvement initiatives by engaging members and prospective members across New York City. This role involves conducting Health Risk Assessments (HRA), documenting health information, connecting individuals to appropriate clinical and social services, and assisting with internal and external program requirements. The position also includes community-based Medicare outreach and enrollment support, educating eligible individuals about Medicare plans and assisting them through the enrollment process while promoting preventive care and improved health outcomes. Key Responsibilities: Conduct Health Risk Assessments (HRA) and document member health information accurately. Engage members and assess needs to support care coordination and follow-up services. Connect members with clinical care, social services, and healthcare resources. Assist with scheduling appointments, educational programs, and transportation services. Enter assessment data into internal systems and maintain accurate records and reports. Participate in community outreach events to educate individuals about Medicare plans. Support Medicare enrollment activities while ensuring compliance with guidelines. Travel across New York City boroughs for outreach and community engagement. Collaborate with internal teams to address member needs and escalate clinical concerns when required. Follow organizational policies and perform additional duties as assigned. Qualifications: NY State Accident & Health Insurance Broker License – Required NY State Life Insurance License – Required High School Diploma or GED – Required; Bachelor’s Degree – Preferred 2 years of managed care or healthcare-related experience Experience with care coordination and diverse patient populations preferred Medical Assistant or LVN experience is a plus Strong customer service, communication, and problem-solving skills Knowledge of medical terminology Proficiency in Microsoft Office and web-based systems Strong organizational, time management, and multitasking skills Ability to work independently in a fast-paced, field-based environment Important Details: 8-weeks contract position Shift is 5x7 Rotating, 35 hrs/week Location is New York City, NY 10004 Hourly Pay Range: $40 - $41 Call to Action: Spots Are Filling Quickly — Apply Now!
