Job Description:
A high-volume, New York–licensed home care agency is seeking a
senior-level Director of Coordination to oversee all care
coordination operations. This role ensures uninterrupted patient
coverage, efficient caregiver utilization, regulatory compliance,
and consistent team performance across the organization.
Responsibilities Include: Oversee all care coordination operations
across the agency Manage Care Coordinators and Supervisors,
enforcing accountability and performance standards Ensure full
staffing of authorized cases and rapid response to callouts and
emergencies Oversee live-in, split cases, overtime usage, and
scheduling adjustments Collaborate closely with HR, Payroll,
Recruitment, Nursing, Intake, Authorizations, and Retention Resolve
escalations related to staffing, caregivers, and patient coverage
Ensure accurate and timely documentation of coordination activities
Track and report key performance metrics including unstaffed hours,
overtime, response times, and complaints Ideal Qualifications:
Senior operational leadership experience in home care or healthcare
coordination Strong knowledge of NY home care regulations and labor
laws Proven experience managing large, high-volume teams Highly
organized with strong decision-making skills Ability to lead under
pressure and resolve complex staffing challenges Salary: $150k/Year
