Job Description:
A Licensed Home Care Services Agency (LHCSA) is seeking an
experienced and dedicated RN to serve as a Field Nurse Supervisor.
This leadership position balances both fieldwork and office-based
responsibilities, overseeing a small team of field nurses while
ensuring quality care and compliance for approximately 50 patients
across Brooklyn and Queens. Responsibilities Include: Supervise a
team of field nurses and support their training and development.
Conduct patient assessments and supervisory visits in-home across
Brooklyn and Queens. Ensure accurate, timely, and compliant
clinical documentation. Serve as a liaison between patients,
families, nurses, and internal departments. Identify clinical and
compliance issues and implement corrective actions when needed.
Monitor patient care plans and ensure appropriate implementation.
Maintain updated knowledge of LHCSA regulations and industry
standards. Ideal Qualifications: Active NYS Registered Nurse (RN)
license. At least 2 years of experience working in a LHCSA setting.
Strong clinical assessment and documentation skills. Ability to
travel throughout Brooklyn and Queens. Prior supervisory or team
lead experience is preferred. Excellent communication,
organizational, and leadership skills. This role is ideal for an
experienced RN looking to transition into a leadership role within
a supportive and mission-driven home care agency. Salary : $110k –
