Population Healthcare Coordinator ColumbiaDoctors is one of the largest multispecialty practices in the Northeast and is affiliated with world-renowned Columbia University Medical Center and NewYork-Presbyterian Hospital. ColumbiaDoctors is looking for a Population Healthcare Coordinator to help us maximize the value of our services, optimize patient satisfaction, and improve health outcomes for our diverse patient population. The Population Healthcare Coordinator contributes to the overall health and well-being of ColumbiaDoctors' patients through data-driven solutions and population health management principles. He/she utilizes cutting-edge technology to assist in the reduction of gaps in care in one of the largest multispecialty practice organizations in the Northeast. This position reports to the Chief Quality Officer and participates in the ongoing development of reports and capabilities required to support the quality and patient safety programs. This position requires the ability to work collaboratively with physicians, staff, and other health care professionals to assist in providing care coordination and closing gaps in care across the health care continuum for all patients within the physician office setting. The successful candidate will be an integral member of the health care team who works to ensure patient safety, best practice, and high-quality standards of care are maintained across the continuum and throughout the organization. Position Activities The Population Healthcare Coordinator works collaboratively with a dedicated team of caregivers to plan, develop, manage, coordinate, and implement clinical integration and population health management services for all payors.Partners with physician leadership and other clinical and administrative staff, as well as external partners and stakeholders, to improve the quality and efficiency of patient services as well as coordinating patient education, health enhancement, and disease prevention activities on behalf of community residents and patients. Collaborates with the ACO Care Coordinators to expand joint efforts to provide a comprehensive care management program to all referred patients.Works with the quality analytics team to move toward a predictive analytics approach to identifying patients and populations for early outreach.Collaborates with payors to utilize claims data and various tools to implement proactive programs to achieve reduction of inappropriate utilization of services such as ER visits, duplicate tests, or procedures, and helps to decrease avoidable readmission rates.Documents in patients' medical records and other data systems to ensure effective data collection and evaluation according to ColumbiaDoctors' Care Coordination and Management Standards.Works with the health care team to facilitate a high-quality patient experience and improved patient health outcomes overall.Manages the duties, responsibilities, and performance of population health coordinators. Skills and Abilities Flexibility, tolerance, and demonstrated ability to take initiative and to prioritize multiple tasks with attention to detailsAbility to function at a fast pace and to interact with a wide variety of people and situations while maintaining a customer focusAwareness of scope of practice boundaries; comfortable seeking direction and assistance from appropriate resourcesExcellent organization and communication skills with customer service deliveryProficiency in Microsoft Office SuiteExperience with use of Electronic Medical Records (Allscripts Touchworks preferred) Qualifications New York State license to practice as a registered nurse, nurse practitioner, licensed clinical social worker, or pharmacistExperience in patient care coordination preferred (ideally more than 2 years)Chronic Care Professional certification preferredExperience with analytics reports such as pursuit lists (COGNOS) is preferredSpanish language fluency is highly desirableAbility to perform all other duties as required by the department and institution All applicants must apply online at: We are an equal opportunity/affirmative action employer.
