Job Description Job Description Position Summary The Social Worker provides comprehensive case management and clinical support to individuals and families experiencing homelessness. This role focuses on housing stabilization, psychosocial assessments, service coordination, and advocacy to support clients in achieving stability, independence, and improved quality of life. Key Responsibilities Conduct comprehensive psychosocial assessments and develop individualized service plans. Provide ongoing case management and short-term counseling to clients experiencing homelessness. Assist clients with housing placement, stabilization, and retention, including coordination with shelters, transitional housing, and permanent housing programs. Connect clients to community-based resources, including medical care, mental health services, substance use treatment, public benefits, and employment services. Advocate on behalf of clients with landlords, housing providers, entitlement agencies, and other service systems. Maintain accurate, timely, and compliant documentation in electronic health record (EHR) or case management systems. Collaborate with multidisciplinary teams, including housing specialists, clinicians, outreach workers, and external providers. Participate in case conferences, supervision, and ongoing professional development. Ensure services are delivered in a trauma-informed, culturally competent, and client-centered manner. Comply with all agency, city, state, and federal regulations and confidentiality standards. Qualifications Master’s degree in Social Work (MSW) required. Knowledge of housing systems, public benefits, and community resources. Strong assessment, engagement, and crisis intervention skills. Excellent written and verbal communication skills. Ability to manage a caseload and work effectively in a fast-paced environment.
