Identifies, diagnoses and treats human responses for actual or potential health care problems for individuals and families. Provides such services as case-finding, health counseling, implementation and evaluation of primary health care as well as management of and referral follow-up of identified care gaps; Plan, organize, direct, coordinate and evaluate primary health needs and addressed urgent/emergent health needs for Ambulatory Care practices at both South Brooklyn Health & Ida G Israel.Specific Duties And ResponsibilitiesThe Nurse Practitioner is charged with and responsible for:

• Perform History & Physical examinations
• Order routine diagnostic testing, e.g., laboratory, radiologic, etc.
• Ongoing medical documentation in the medical record, e.g., progress note, etc.
• Refer for appropriate consultation (s).
• Address identified patient care gaps and refer to additional imaging or specialty care as needed.
• Order medications and write prescriptions subject to Section 6902 (3) (b) of the New York State Education Law.
• Perform minor procedures and diagnostic testing, e.g., EKG, IV, phlebotomy, injections, NG tube, urinary catheterization, suturing, cultures, etc.
• Education and counseling of patients and families.
• Perform more advanced procedures and ACLS with documentation of appropriate training and documentation of competency.
• Medication refills
• Other duties as required within the scope of practiceMinimum Qualifications1. A valid New York State license and current registration to practice as a Registered Professional Nurse issued by the New York State Department of Education (NYSED); and
  • Current certificate as a Nurse Practitioner issued by the NYSED; and
