ACS Service Coordinator

Job Description:

Position Summary: The ACS Case Coordinator will provide case management services to families involved in a Homemaker Service Program, to stabilize the home and maintain family cohesiveness thereby averting entry into the foster care system. The Case Coordinator will schedule, coordinate and supervise the activities of the homemakers, conduct home visits, establish case goals, conduct case conferences, maintain case records and make referrals to outside agencies for ancillary services as needed.

Essential Duties:

  • Conducts initial visit to the client prior to or concurrent with initiation of services which must begin within 72 business hours of referral
  • Schedules aide/homemaker staff for client service matching client care needs with skills of homemakers
  • Generates initial contact calls with new clients to verify information, set initial evaluation visit date and establish a client record
  • Conducts home visits to each client every 6 weeks to assess progress, supervise the HCA, and review the Homemaker Task/ Care Plan and revises Task/Care Plan as necessary
  • Notifies ACS when a case cannot be staffed with a homemaker
  • Attempts three log calls weekly and notifies ACS when attendance cannot be verified during at least one of those attempts
  • Ensures only authorized services are provided as stated by ACS
  • Maintains case records documenting all problems and incidents as required by contract, especially with regard to child care, immediately reports any serious problems or abuse of children to immediate supervisor, Central Registry and to ACS
  • Reports all incidents or suspected incidents of child abuse and/ or neglect whether physical, sexual or psychological
  • Maintains weekly staffing schedules and revises with correct information as cases and assignments change
  • Receives authorizations, change of hours forms and discharge forms, confirms accuracy, changes and documents client file
  • Assists with in-service training sessions
  • Maintains a high degree of confidentiality at all times due to access to sensitive information
  • Maintains regular, predictable, consistent attendance and is flexible to meet the needs of the department
  • Follows all Medicare, Medicaid, and HIPAA regulations and requirements
  • Abides by all regulations, policies, procedures and standards
  • Performs other duties as assigned 

Position Requirements & Competencies:

  • BSW Degree or BA degree from an accredited university; with a minimum of one year experience in casework.
  • Bilingual proficiency in Spanish and English, preferred
  • Ability to work independently
  • Excellent verbal and written communications skills
  • Ability to effectively analyze, interpret, apply and communicate policies, procedures and regulations
  • Ability to operate PC-based software programs including proficiency in Word, Excel, and PowerPoint
  • Must have reliable transportation
