New York City, along with housing in upstate New York and
Connecticut. Supportive housing - affordable housing paired with
services designed to help people maintain their homes for the
long-term - is widely recognized as a proven and cost-effective
solution to chronic homelessness. But we didn't stop at providing
housing. Breaking Ground's programs and services help people
experiencing street homelessness - especially those who have been
on the streets the longest - to come indoors. Through our Street to
Home outreach program and transitional housing resources, we help
people get, and stay, on the path to a permanent home. One of
Breaking Grounds programs isresponsible for
delivering24/7,comprehensivehomeless outreach servicesin the area
spanning 31 st 34 th Streets from 6 th 9 th Avenues
(PennDistrict)inMidtown Manhattan.Reportingto the Program Director,
the Social Services Supervisor will provide oversight and support
tonineOutreach Case Managers.Utilizing harm reduction and Housing
First approaches,this outreach programwill be directly responsible
for responding topotential homeless conditions and providing
interventions appropriate to the situation.TheSocial
ServicesSupervisorwill engage with individuals presenting as
homelessand willcreate individualized plans that aim to address the
housing, healthcare, and other needs of the individual.Where
appropriate,the Social ServicesSupervisorwill collaborate with
other homeless service providers throughout the citytoavoid
duplication of services and help individuals overcome homelessness.
This position has a 35-hour work week, with some flexibility.
ESSENTIAL DUTIES:
- Provide clinical direction and direct supervision
(toincludescheduling, supervision of client cases)
fornineOutreachCaseManagers,ensuring timely and effective response
to homelessconditions
- Serve as the primary liaison with community partners and in
close collaboration: help to develop and implement processes and
protocols for responding to, and following up on homeless
conditions and to provide education and training to various
communitypartners
- Create strong working relationships with clients and Breaking
Ground coworkers, specifically colleagues in the Street to Home and
Safe Haven programs
- Develop strong collaborative relationships with corporate
partners, NYPD, EMS, and otherstakeholders
- Attend meetings onsite with community partners and at
BreakingGround
- Ensure appropriate, accurate, and timely documentation of
outreach activities and ensure weekly reportsubmissions
- Perform other related duties asassigned MINIMUM
QUALIFICATIONS:
- Experience with the street homeless population
stronglypreferred
- Willingness to engage homeless individuals in a variety of
settings both indoors andoutdoors
- Understanding of harm reduction required
- Housing-based case management experience stronglypreferred
- Minimum of two years supervisory experiencepreferred
- Ability to positively engage with clients who have significant
mental health issues, current substance use, and often medical
illness and cognitiveimpairment
- Superb collaborative skills: ability to work successfully with
a wide range of internal and externalstakeholders
- Ability to communicate (verbally and written) with diverse
populations andstakeholders
- Bachelors degree, LMSWpreferred
- Excellent computer skills, including proficiency in Microsoft
Word and Excel. Familiarity with Access-based databases and the
ability to learn new programs arepreferred
- Bilingual Spanish/English is preferred
- Valid NY Drivers License in good standingrequired
- Travel throughout New York City (the five boroughs) as needed
to collaborate with servicepartners Consistent with NYCs mandate,
all Breaking Ground employees, interns, volunteers and

accommodation. Compensation: $30.21 - $30.21 / Per Hour
Compensation: $54,982.20 - $54,982.20 / Per Annum
