Position Description
The Program Coordinator, TGNCNB Programs will assist with the
coordination and implementation of all group level interventions
for this new multi-year grant dedicated for Transgender, Gender
Non-Conforming and Non-Binary community members. The individual
will be responsible for the day-to-day coordination, recruitment,
and engagement of clients for these interventions. The Program
Coordinator will plan and provide direct facilitation as well as
support clients with linkage and navigation services.
Additionally, the Program Coordinator will also be responsible for
establishing and maintaining professional work relationships with
the Prevention team, community leaders and stakeholders; including
Transgender, Gender Non-Conforming and Non-Binary community
members.
Essential Job Functions
The following duties are mandatory requirements of the job:
- Conduct screening and intake sessions for potential group
participants
- Assess, tailor and adapt the Seeking safety intervention
curriculum to fit the needs of our clients
- Coordinate with the Peer Navigation Specialist for recruitment
of new participants and support their retention
- Lead facilitation of the weekly psycho-educational Evolution
group
- Supervise and train Peer Navigation Specialist to co-facilitate
the interventions.
- Oversee HIV Navigation Services (HNS) or Case Management
Services including reviewing and completing the proper
documentation needed via the funders.
- Increase client understanding and awareness of HIV prevention,
care and support services, and build self-efficacy skills for
navigation of these services.
- Increase client awareness of the importance of Mental Health
and well-being
- Provide group participants with referrals to available services
as necessary
- Collect and report required data from group activities,
including dissemination of client incentives Other
Responsibilities
The following duties are to be performed as assigned by the
supervisor:
- Assists with special projects and perform other related duties
as assigned by supervisor
- Adhere to workplace policies and procedures, including
confidentiality, documentation, channels of communication,
workplace culture, dress code, and conflict resolution
- Meet periodic program reporting and departmental deadlines in a
timely manner Education and Certification
- Bachelor's degree preferred or equivalent combination of
education, training and experience. Areas of study in the following
field preferred: design, community health education and/or social
sciences Special Skills and Knowledge
In addition to the above-listed job responsibilities and
educational requirements, the ideal candidate for this position
possesses most or all of the following:
- Excellent written and interpersonal skills
- Specialized skill in risk behavior assessment and behavioral
change interventions and techniques
- Thorough knowledge of social, health/medical and mental health
and wellness service systems, including treatment center programs,
and the criminal justice system
- Willingness to work within a harm reduction model of HIV
prevention
- Proven knowledge and experience of key computer programs
including Microsoft - Word, Excel, PowerPoint, Outlook, and
Internet Explorer
- Demonstrated experience in seeking external support and
guidance on complex projects to provide rigor and expertise
- Proven experience at implementing and adhering to planning
documents which highlight work plans and areas of focus
- Study and short course work in community education, community
or public health, and harm reduction are highly desirable
- Proven experience in facilitating timely implementation of EBI
groups, crisis management and risk behavior assessment
techniques
- Proven experience in facilitating short-term psycho-educational
groups with Transgender, Gender Non-Conforming and Non-Binary
community members
