Inc. (NAC) is an award-winning not-for-profit agency in Midtown
Manhattan dedicated to serving children and families with medical
complexity, chronic health conditions, significant disabilities,
and behavioral challenges. To assist in achieving this mission, NAC
is currently seeking a Case Planner to join its Specialized Family
Foster Care Department. As a Case Planner in NAC's Specialized
Family Foster Care Program, you'll play a crucial role in assisting
us in achieving our mission. We're seeking a motivated and
compassionate professional who can bring fresh ideas and a
dedication to making a difference. Title of Position: Case Planner
(Bachelor & Master Level), Specialized Family Foster Care Position
Type: Full Time Pay Range: Bachelor- Level: $55,000 - $60,000.
Additional compensation for bilingual skills | Master- Level:
$65,000 - $70,000. Additional compensation for bilingual skills and
licenses (LMSW, LMHC, etc.) Department: Specialized Family Foster
Care Description of Responsibilities: Provides casework and
clinical services to children and families utilizing individual,
group and family modalities. Conducts assessments of and
coordinates services for a caseload of 11-12 foster children as
recommended by NAC's multidisciplinary treatment team. Conducts
home and community based visits to families; supports parent/child
visits both in person and virtually. Works collaboratively with
psychiatrists, psychologists, pediatricians, nurses, education
specialists and fellow clinicians in a dynamic, strength-based
interdisciplinary team model. Prepares for and participates in
Family Team Conferences and NYC Family Court proceedings. Completes
ACS and NAC paperwork in a timely manner and ensures that
documentation conforms to Agency, City and State regulatory
requirements including but not limited to progress notes, court
reports and the Family Assessment and Service Plan (FASP). Makes
adequate use of individual and group supervision, on-site/virtual
clinical trainings and mental health in-service. Required Education
and Experience: Bachelor Level Case Planner: BSW, Bachelors in
psychology, human services or a related field. Master Level Case
Planner: Master's degree in social work, mental health counseling,
human services, psychology, public administration, or a related
field. Prior experience in foster care, child welfare, and/or
health care preferred Required Skills: Excellent written/verbal
communication and organizational skills Strong engagement skills
Proficiency in Spanish is preferred (and includes a salary
