looking to hire a Case Manager to serve as part of the Pathways for
Access to Careers and Employment Workforce Development Program
(PACE) Team. The approach of the PACE Program emphasizes that there
is no one-size-fits-all solution for clients needing education and
or training support. Career Team is excited to work with an
enthusiastic Case Manager who will assist PACE clients to provide
professional, intensive case management to help guide clients in
career development and training/educational pathways that lead to
gainful employment and self-sufficiency. This position is expected
to be 100% onsite. The Case Manager is an integral part of our team
and will assist us with our mission of accelerating the human

TEAM’s Success : Manage a portfolio of clients by motivating their
education, training, and job search efforts, which include coaching
through barrier removal, professional development, and ultimately
job placement that leads to self-sufficiency Employ a
human-centered approach to case management, acknowledging that each
client who comes in has unique needs, abilities, and circumstances
Develop and manage a comprehensive working relationship with all
stakeholders, including clients, governmental agencies, employment
partners, service providers, etc. Identify and assist with
mitigating barriers to program participation, including but not
necessarily limited to health, mental health, housing, childcare,
and domestic violence issues Maintain an updated client database
and track client engagement in services, and accurately document
case management activities in the State database daily Coach and
mentor clients through personal and professional development
through workshops, Career EDGE, and appointments Prepare reports of
activities and achievements Approach each day and task with a "ZAG"
mindset Performs other tasks as assigned The Ideal Qualifications
for this Position Include: Bachelor’s degree in social work, social
services, counseling, business administration, or related field, OR
a minimum of 2 years of experience in workforce development-related
case management services with a professional certification
Experience working with populations similar to the PACE program’s
clients, demonstrating an understanding of their unique
circumstances Prior experience developing, evaluating, and
implementing milestone-driven service plans Ability to identify
community resources/services and connect clients to these resources
Strong interpersonal skills, including the ability to build
trusting relationships Effective communication, listening, and
coaching skills Multilingual proficiency, preferred Proficiency in
Microsoft Office and other relevant software Ability to submit
accurate written forms and reports Exceptional time management
skills to meet deadlines and work under pressure A Valid Driver's
License Salary : $55,000/yr Employment Type : Full-Time, Non-exempt
